PSI Concern & Ethics Reporting Form

This form provides a channel for students, instructors, and community members to report ethical concerns, boundary violations, institutional issues, or concerns related to PSI training activities.

Instructor roles are limited to curriculum delivery and training facilitation. Institutional oversight, ethical review, and organizational concerns are the responsibility of the PSI Leadership Committee (LC), as such, reports submitted through this form will be reviewed by the Leadership Committee.

Note: Anonymous reports are welcome. However, anonymity may limit the Leadership Committee’s ability to follow up, gather additional information, or fully investigate the concern.

Confidentiality Notice

Information shared through this form will be reviewed by the PSI Leadership Committee and kept confidential within the committee. If contact information is provided, the Leadership Committee may follow up for additional information.
